Person Centered Planning

The Person Centered Plan (PCP) is a document that outlines the interests, goals, strengths, and challenges of the participant. It creates an image of their many unique needs and abilities. The PCP is created through the input of the participant, their caregivers, and anyone close to the participant who they would like to include.

This document is created by an Independent Facilitator certified in Person Centered Planning. It is used to determine which services will best fit the participant’s needs and desires within the Self-Determination Program.

Step 1: Initial Consultation

A brief phone call to gather basic information about the participant and their current regional center services. This consultation will establish a partnership between the participant and Ownself Facilitation.

Step 2: Interview

An in-depth virtual interview with the participant, caregivers, and select attendees to learn about the strengths, challenges and needs of the participant. The PCP document will be created based on the information gathered.

Step 3: Review

The created Person Centered Plan is reviewed by the participant and caregiver to ensure accuracy and provide additional input for revision. Multiple reviews may be required until the PCP is fully satisfactory to all invested parties.

Step 4: Submission and IPP request

Once the PCP is complete, it will be submitted to the participant’s Service Coordinator, and an Individual Program Plan meeting is requested. This will move the participant into Pre - Enrollment Transitional Services.
